When you verify your account in to the patient portal, you’ll need to upload a picture of your valid photo ID (e.g. driving licence or passport), proof of address dated within the last 3 months (e.g. a bank statement) and finally a clear photograph of yourself that we can compare with your ID, showing a picture of yourself.

Accepted forms of ID are:

• Valid Passport

• Valid UK Driving Licence

• Valid Biometric Resident Permit

• Valid National ID card

Accepted proof of address are (dated within the last 3 months):

• Valid UK Driving Licence (if not used for ID)

• Bank, Credit Card or Mortgage Statement

• Utility bill

• Mobile phone bill

• Council Tax bill

• Benefits Letter

• HMRC/National Insurance letter
